I proofread a magazine which is aimed at high-earning professionals.
The thing that drove me mad was that they missed out an apostrophe in their tagline on the front cover of every issue. I approached the editor about it and he told me that it was a deliberate omission as they didn’t like the way it looked on the design! I was quite embarrassed to have my name mentioned in the editorial credits, but thankfully, they have now changed the words and no longer require an apostrophe.

I thought the usual stuff was a saved template. Either on computer and/or already set at the printers and would have to be purposefully changed – not mistakenly created.
Maybe it’s was a disgruntled employee or “inside joke” – New SS, like a neo-fascist paper.
